Please include the following information:
- The name of our customer (who our insured is).
- The name of the certificate holder.
- Certificate holder address.
- Method of delivery:
- Email address where the certificate needs to be sent (Preferred)
- -OR-
- Fax number where the certificate needs to be sent
- A call back number and contact name in case we need to reach them.
Please Note:
If during normal business hours [8:30 a.m. – 5 p.m. CT] you have not received your certificate within one hour please contact our office at once.
